Whiplash symptoms can vary

Whiplash-associated disorder can include neck pain, stiffness, headache, shoulder discomfort and difficulty with driving, work or sleep. Symptoms may begin immediately or become clearer over the following day.

Assessment considers the crash, symptoms, movement, neurological signs, prior health and any features that require medical investigation.

When to seek urgent care

Seek urgent medical assessment for worsening weakness or numbness, severe or unusual headache, loss of consciousness, confusion, visual or speech change, difficulty walking, repeated vomiting, chest pain, breathing difficulty or severe pain after major trauma.

Early management

  • Clear explanation and reassurance based on the assessment
  • Comfortable neck and upper-body movement
  • Maintaining safe everyday activity
  • Short-term modification of aggravating tasks
  • Advice for work, driving and sleep where relevant

Progressive rehabilitation

Exercise can progress from movement and low-load control to strength, endurance and the demands of work, driving or recreation. Manual treatment may assist some people but should support, rather than replace, active rehabilitation.

If recovery is slower than expected

Persistent high pain, sleep disruption, fear of movement, low activity and work difficulty can affect recovery. Regular review helps identify barriers and adjust the plan. Further medical or multidisciplinary support may be appropriate when progress is limited.
